Baldur’s Gate 3 has plenty of crucial items for you to keep track of, but the Netherstones are without a doubt something you don't want to lose, as you need to collect all three of them from Ketheric, Orin, and Gortash to beat the game. Despite being extremely important, Baldur's Gate 3 allows you to drop these stones wherever you like, and you're almost always rightfully scolded by The Emperor for dropping them willy-nilly.
Should you somehow drop one and lose it forever, your stupidity is often greeted with a game over cutscene of the Elder Brain turning your character into a Mind Flayer. There are rare occasions on which The Emperor will pop your Netherstones into your camp's chest for you, such as if you drop them in the Githyanki stronghold and enter Act 3, so we know the game does have a few fail-safes to ensure it's beatable.
Walking away from a beloved masterpiece like this is its own challenge.
