At the beginning of Baldur's Gate 3, our companions are quite surprised to find that they are not showing signs of ceremorphosis. The game opens with everyone being given a tadpole, after all, so what should follow is a week of excruciating pain as they're transformed into mind flayers.

You later find out that there is a reason that you and the rest of the gang are unaffected while others aren't as lucky. However, if you actually want the threat of ceremorphosis hanging over you, then we have the perfect mod for you.

Countdown to Ceremorphosis by Nexus Mods user Cheek is exactly that - a mod that kicks off the timer until your character (and everyone else with a tadpole) is turned into a mind flayer. This gives you seven in-game days to beat Baldur's Gate 3's story, and even debuffs you each day to reflect the stages of ceremorphosis. If you don't defeat the Netherbrain in time, then all of the tadpole'd companions turn into mind flayers, and you get a game over.
