Actor Cariana Conti, who plays Isabella in The Old Country, broke the news during an appearance on a Bubbly Nuggets livestream. A viewer asked if there would be more Mafia games in the future, to which Conti replied yes, confirming that a new Mafia project had already been greenlit at Hangar 13, implying work is either already underway or development will begin soon.
Considering how well received The Old Country has been, and the success of the Mafia series as a whole despite its hiccups, it's not exactly a shock that 2K would want more of it.
