It’s largely the same hilarious, gory surgery sim you know and love, but its mechanics don't translate well to VR.
As it stands, you should absolutely hold off on Surgeon Simulator: Experience Reality for PlayStation VR until the controls are overhauled in the planned update. I ended up having some fun once I learned how to work around those glaring issues to a certain extent, but even in the best of times, it was a struggle. It's a shame the launch went this way, because there's an entertaining VR experience buried in here.
To put it bluntly, sitting through a Tijuana back-alley dentistry that would be more enjoyable than this disaster. Unless a major patch hits soon, this is a procedure that has graduated from terminal to dead on arrival.
Far less fun than both it could and should have been, Surgeon Simulator ER is undone by poor PSVR implementation and lack of additional content over the previous non-VR release.
It's the same game, but in VR. If you didn't like the game before, you won't like it now. It's less frustrating, but a fun little party game to play with friends.

Without any new content from the last release, broken controls, and an absurd amount of glitches, Surgeon Simulator: Experience Reality is really the one that needed surgery.
